Why don't I see my screening questionnaire?

Solution for if you are unable to see your screening questionnaire on your dashboard

There can be several reasons for why the survey is not showing up on your dashboard:

  1. You were added recently to the system. It takes a day to activate the survey, so please check with your admin if you were added a day before you were requested to take the survey.
  2. You may have been assigned to the wrong site. Please check with your admin if you were added to the right department/site.
  3. You may have missed the survey period (i.e. you may have signed in after the survey had expired for that day). Please check with your admin if you have any active assessments to be completed. 
  4. You may have already completed the survey for that day. Please check to see if you have completed all the assessments.
  5. If you are a parent, your child's survey may not have been delegated to you. Please check with your admin if your child's account was mapped with you or your spouse.
