Here is the procedure to fill out a new certificate
Submitting a Certificate:Click New Certificate on the Certificates tile to submit a new certificate, and we can use various filters
Once you select New Vaccine Card, you will be asked to select a card type. The card types and their descriptions can be found on the diagram below.
Once you’ve chosen your card type, you will be brought to the corresponding New Certificate page.
Clicking the plus button under Primary Vaccine Series brings up the Vaccine Does Information window.
Clicking the Add Attachment button to the right of Attachment brings up a window to submit attachments
Clicking the 
to the right of Attachment brings up a guide message of attachment
